FIT TEST VICTORIA SERVICE TERMS & CONDITIONS
These terms and conditions apply to onsite group fit testing services. Please review these carefully. By booking fit testing services with us, you are acknowledging that you have read, agree to, and will abide by these T&Cs. A link is provided to this T&C page in all booking confirmation emails.
‘We’ and ‘us’ refers to Fit Test Victoria Pty Ltd. ‘You’ and ‘your’ refers to the client.
1. General Considerations

You acknowledge that the respiratory protective equipment (RPE) you have selected is fit-for-purpose and compliant in line with AS/NZS 1715:2009. The RPE is reasonably comfortable and acceptable for the wearer.

You acknowledge that all participants are medically and psychologically fit to safely wear a respirator and get fit tested. If there is any question regarding medical or psychological suitability, or if any wearer requests it, a medical clearance is required. If a participant alerts us to a medical or psychological condition that may preclude them from safely wearing a respirator, we reserve the right to forgo fit testing on that person.

You agree that all participants will be clean-shaven at the time of testing and anytime they are required to wear a tight-fitting respirator in line with AS/NZS 1715:2009, OSHA 1910.134, WorkSafe Victoria, and Safe Work Australia. Some mustaches are acceptable. Click here for a guide.

We will not be responsible for informing participants on the day of testing that they must be clean-shaven to wear a tight-fitting respirator. This usually creates a lot of grief and angst for those with a beard. Participants must be informed before they attend their test of this requirement. If participants arrive with facial hair, they will not be tested.
2. Participant Requirements

All participants must review the 'How to Prepare' document before attending their fit test appointment. These will be provided to you as a PDF when you secure your booking.

Participants must come to their appointment clean-shaven. We will not test anyone with facial hair as this is against WorkSafe requirements, AS/NZS 1715 standards and OSHA protocols. Facial hair prevents tight-fitting respirators from achieving an adequate seal. If there is a compromise in the seal, then the wearer will not be adequately protected from airborne hazards which can be silica dust, asbestos, gases, vapours and/or fumes.

It is the responsibility of the person booking fit testing services to ensure that all participants are ready for their test.
3. Bookings

If you choose our 'per person' option, you must let us know how many people need to be tested per day at each of your sites so we can plan accordingly.

The minimum number of participants for onsite testing is 5 per site. If you have less than 5 people to be tested, we may still be able to come to you. The minimum charge, however, will be for 5 people. We can, at our discretion, require a higher minimum for onsite testing based on your location and travel requirements.

If you choose our 'day rate' option, we will be at your site for a set number of hours.

To ensure smooth operations on the testing day, we recommend that you use our online booking system. We will send you a link that can be shared with staff so they can book themselves in at a specific time on the testing date. Alternatively, you (or your admin team) may opt to book staff at times that are conducive to your work schedules and processes. If an admin is booking on behalf of the staff, staff must be made aware of their appointment times and pre-test requirements.

Once you secure a booking with us, you are agreeing to all the terms and conditions outlined here.
4. Pricing & Invoicing
Per Person Pricing

Pricing is based on the number of people you schedule to get tested. Discounts may apply for groups. You will be provided with a quote that must be accepted to secure your booking.

The price per person includes testing of ONE type of mask. For example, a reusable or a disposable. If you need testing on more than one type of mask, additional charges apply per our pricing guide.

Discounts are based on the number of people being tested per booking. One booking may be on a single day or over multiple days depending on how many people need to be tested.

We do not provide group discounts based on potential future bookings. For example, if you schedule 15 people for fit testing and note that you will have another 20 people who need testing in the future, you will receive the discounted price for the 15 you are booking now.

You will be invoiced for the number of people you book and the price you are quoted. For example, if you schedule 20 people and only 12 attend, you will be invoiced for 20. Once you secure a booking, we assign fit testers and resources to be at your site for the time it takes to test the number of people requested. If you need to adjust numbers, you must give us 24 hours or more notice.

If someone can't attend because they are unwell, or have extenuating circumstances, we may allow them, at our discretion, to attend a make-up test at the Fit Test Victoria offices.

You must ensure your staff are aware of their fit test date/time, are clean-shaven, have not smoked/vaped 30 minutes before the test, and have read the 'How to Prepare' document.

Day Rate Pricing

You will be invoiced for day rates based on our pricing guide. The pricing guide will be provided upon request.
Masks
We test the respirators that you are currently using. Please have these ready on testing day.
Reusable Respirators

If you don't have reusable respirators, you can borrow ours for your staff to test on at no cost. We will let you know which brand/model/size staff pass on. You can then purchase masks from us or your usual supplier.
Disposable Respirators
- If you don't have disposable respirators, you can purchase them from us by the box for testing. We sell a variety of brands such as 3M, Maxisafe, and Prochoice.

If you do have disposable respirators and staff fail on the ones you have, we bring along a variety of brands/models/sizes to try out. If we use our masks because staff fail on yours, you will be charged per mask used.

5. Purchase Orders and Quotes

You will receive a quote for either per person or day rates depending on which option you choose. Quotes must be accepted to secure your booking.

We request purchase orders for all services as this makes billing, accounting and payments much easier for both of us. If you do not provide us with a PO, the quote will serve as the PO.
6. Cancellations
We charge cancellation fees to account for staffing arrangements and the allocation of resources that have already been committed. Last-minute cancellations occur often and disrupt our operations, impacting our budget and preventing us from efficiently managing our services. This policy ensures we can maintain high-quality service to all our clients.

Cancellations made 48 hours or more prior to the testing date will not incur any fees.

Cancellations made less than 48 hours but more than 24 hours before the testing date will result in a cancellation fee equal to 25% of the quoted price.

Cancellations made within 24 hours or less of the testing date, including same day cancellations, will result in a fee equal to the full quoted price. Please note, if you have made a last-minute booking and it is already 24 hours or less before the testing date, you will be invoiced for the full quoted amount.
7. Payment Policy

Unless otherwise agreed upon by us and you, payment is expected within 30 days after you receive an invoice.

Late payments over 30 days will incur a late fee of 2%.

All late payments after 60 days will continue to incur a late fee of 2% per month of the total owed (including any previous late fees).

If a collection agency must be used to obtain payment, the costs associated with the collection agency will be billed to the client.
